Data Separation Emergency Fix
Remove all cross-organization and orphaned data
Nuclear Cleanup - Remove ALL Orphaned/Cross-Org Data
This operation will:
- Delete ALL documents without org_id (orphaned)
- Delete ALL attachments without org_id (orphaned)
- Delete documents where org_id doesn't match parent requirement's org_id
- Delete attachments where org_id doesn't match parent task's org_id
This is PERMANENT and cannot be undone!
